What to Bring
What to bring to your first visit:
- Current health Insurance Card
- Valid ID
- Updated immunization record
- Completed New Patient forms
- Any available interim records from any urgent care clinic or hospital
- Copies of any interim laboratory or diagnostic reports from other treating providers
- Any paperwork or forms you need filled out
- List of all current prescription medications and over the counter medications that you are taking
